I'm in California, & to the best of my knowledge followed the indoor masking guidelines.
Indoors (public places)? Absolutely!
Outdoors? Hell no. Why would I? That would be silly!

In Austria had by law wear a FFP2 mask in public transport, shops and working place. Made about 30 hrs a week. Didn't get covid, and envy my 5 co-workers who did get covid very mildly probably with slightly more hrs per week (full-time employed) than me.
Beside this poll not covering the really important fraction of those who died from covid, nor indicating if the sniffle most get from covid was really worth the whole hussle - it is set for its results being completely meaningless even for mild covid.
Because from the results we can't know if the persons with covid ever wore a mask, which kind or under what circumstances?

What mask did you wear prior to being fully vaccinated or contracting COVID-19?
Question 2 at the outset seems to wrongly exclude all not vaccinated or free from covid. At least here the anwer options should have been doubled each, with 'diagnosis' or 'without diagnosis'.
For those who WERE diagnosed with COVID-19 (Positive PCR test only):
Even with this question, where an association could be concluded from, only about the believe-system is asked, not actual compliance.

seivtcho suggested a poll, yes, but seivtcho didn't suggest the question are you vaccinated lol
and the most of the other questions.
The aim of the poll that I suggested is to be seen how many of the infected and now many of the non infected weared a mask.
The results of that poll can't be statistically examined for answering that question.
Manybe an option would be to be made two polls - for infcted and one for non infected
then you may say - the infected answered this, the non-infected - that.

I noticed, that there is one yes for the question "Have you ever been diagnosed with COVID-19? (Positive PCR test only)"
And two "Never wore a mask if I could avoid it"
my question is, does the one of the Never wore a mask if I could avoid it belongs to the yes in the Have you ever been diagnosed with COVID-19? (Positive PCR test only)

Unfortunately the polling system here doesn't seem to (or it was not obvious how to) allow for a branching survey, where questions are based on answers to previous questions.
There probably should be a question with a lot of possible answers like:
a. I never wore a mask, went out in public a lot, and was diagnosed with COVID-19
b. I never wore a mask, went out in public a lot, and and have not been diagnosed with COVID-19 (and never felt sick and avoided taking a PCR test)
c. I always wore an N95 mask and was diagnosed with COVID-19
d, etc. etc.
