Well succesfull plastic surgery doesn't mean that person will look like he's "surgically altered". I mean - young looks isn't a "mystical" thing at all. It consists of a number of factors. Like - some persons eyelids may start sagging/drooping - remove the excess skin and voilla - he's back to the "state" he was before that happening. It's really funny if somebody would say "Oh, he/she looks her age, because... he/she had that "work" done.". That's not how it goes - they look young or they does not, no matter what lenghts they went to achieve that.
If the surgery is very successful and people can't tell, then the person would look naturally young. Otherwise it looks like a face that underwent surgery so most people would know better than to take years off the person's age.
