Which is really better and why? They seem to do similar things and wondered which is best for arterial health? My father has vascular dementia - thickened arteries in the brain.

They have very different properties, so it's kind of like asking which is better, apples or oranges. In both cases, it depends.. Vitamin K has effects on calcium homeostasis- in particular, it helps it to stay in the bones where it's needed and out of the vasculature and other soft tissue where it isn't wanted. Vitamin E is an antioxidant. Given a choice between the two, I'd give him K and add a mitochondrial antioxidant like c60 olive oil or MitoQ.

Which ever way you go, just be careful of interactions with medications. For instance, if he's already on a blood thinner, Vitamin K may be a bad idea. Similarly high doses of Vitamin E can interact with a variety of medications and seems to have unexplained bleeding issues in some people. See:

The above abstract suggests to me that E and K are not a good idea to take together. If I had to pick one, I'd go with K.
Olive Oil and/or C60 might be the path of least interactions. I haven't seen much research on c60 and dementia, just a few Russian abstracts. But there are a number of favorable olive oil studies. Here's one:

Personally, I'd throw the C60 in too. If the anti-oxidant properties of Olive Oil are responsible for the good dementia results, c60 might be even better. Also, keep in mind that c60 and K both benefit from fats for uptake so if you take either or both of them together with olive oil, it should be win-win.

Olive Oil contains both vitamin K and vitamin E, and plenty of things that are probably more beneficial than either of them. .
Eat Olive Oil, and green things. Then there are many herbal polyphenols from extracts that can enhance autophagy, they may be found in Ginkgo Biloba, Ginseng, Red Wine Extract (resveratrol + stillbenes), Curcumin, Gotu Kola, etc. Then there are herbal mushrooms (and extracts) like Red Reishi, The Lion's Mane, Maitake, etc.
I wouldn't personally advise to take all those extracts, too many things potentially may interfere with each other. I personally rely on Ginkgo Biloba and the three mushroom extracts I mentioned. Then you have to check medicines etc. that he is on for potential interactions.
