I lost 48.4 pounds ( 22 kilos ) in 1 years due to gastritis / sibo / ibs and i am in desperate need to gain back some of it..

I have lost nearly all my body fat and reserves and when i look to myself in the mirror its like a whole other person and its costing psychically and mental its toll.

Because my stomach reacts to almost everything i put in i cant gain any weight and i am looking for something with much calories but easy to digest.

I have all ready tried nutritional drinks like Ensure and Vital, but due the Maltrodextrin and chemicals they make my IBS worse.

Due to IBS weight gainers are also a no no.

If someone has some advice on to gain weight this would me much appreciated!>

You have to find first a solution to your health issues or anything else will be totally unsuccessful.

BPC157 orally could be helpful, I suggest to do some research about it.

Avoid gluten.

Maybe try low carbs

Fats are high calories, try add some coconut oil and olive oil.

If not already start training, take it easy at first, stress plays a big role here.

Mate I have been in your position. I have colitis and have for a decade or so. I could gain weight with great effort but I would lose it as soon as I stopped force feeding myself.  Also gaining weight with IBS is a terrible experience -- you will be farting and shitting constantly -- god forbid you have diarrhea or constipation, which you probably do.

My issues got a lot better lately and as I result I gained a bit under 10kg entirely by accident.

You need to just worry about your health problem and the rest will get better once you do. Forget cosmetics. I wish you luck.
